HEART Community and Adult Day Service

 

At HEART (Helping Every Adult Refine Transition), individuals with disabilities voluntarily enroll in a program designed to help them access educational and career-based opportunities. These opportunities are no longer available to them due to aging out of school-aged services.

HEART is an initiative of the Muskingum Valley Educational Service Center (MVESC), reflecting its commitment to supporting individuals with disabilities in achieving their goals.

 

 

Program Location

The HEART program is located at:

205 N. Seventh Street, Zanesville, OH 43701

This facility is shared with the Muskingum County Health Department and Muskingum County Child Protective Services. Situated in downtown Zanesville, the location provides individuals with convenient access to public transportation, local facilities, and work opportunities.

Participants travel to and from the program via a combination of public and family-provided transportation. Public transportation options, including services available through the SEAT website, ensure accessibility for all participants.

 

Daily Schedule

Participants make the independent decision to attend the program daily, which runs from 7:00 AM to 3:00 PM on weekdays. Participants return to their place of residence each evening and arrive again in the morning to engage in activities focused on their transition needs.

Services and Support

  • Job coaching to secure and maintain employment, vocational rehabilitation for skill development, and person-centered planning to align goals with individual aspirations.
  • Direct teaching and practice of daily living skills such as grocery shopping, self-care, and social skills, all embedded through community involvement and exposure.

 

Accessibility and Transportation

Participants benefit from a combination of public and family transportation options.

Public transportation can be coordinated through the SEAT website, allowing for reliable and flexible travel to and from the program.
